They could work similarly as they do in many email clients (Thunderbird, Evolution etc.).
There would be an additional pre-set folder, called Labels, each in its one chat.
The labels could be created and edited in Settings. This would involve giving them a name, color and possibly icon/emoji.
In each chat (including private, secret, public groups, channels and saved messages), an additional option for each message to be marked according to the chosen label, with multiple options possible.
All labels would appear in the Labels folder in their own chat as if they were a chat themselves (eg. all messages marked with the label Follow up, would appear in the folder Labels within their own chat called Follow up. The same would apply to other labels.
In the Lables chats, messages could be pinned and replied to without being redirected to the original chat. That way, replies would automatically be marked with the label of the chat (eg. Follow up).

I also don't understand why you talk about "deep change". Telegram already has the ability to file chats and messages under folders. So I don't see the big change here but just an useful organizational improvement.
Just to give you few examples on our tags are in no way useful in some situations:
- With the tags, you can only mark your own messages. You can't edit somebody else message. If one (or more people in a group chat) sends multiple messages after the one you wanted to tag, you'd lost the opportunity to even tag it yourself by the cumbersome way of adding a reply with the tag yourself.
- Messages in DM (one-to-one chats) cannot be edited after 3 days, denying you the possibility to organize your messages if not done withing that time frame.
- Tags are more difficult to follow. If you are discussing multiple topics and you want to tag one particular to follow, once recalled with the tag (let's say "#Follow-up") you won't get all those messages together but you'll have to jump them in the chat. This is even more tricky on a mobile where you don't even have a separate column for the tags.
There are even more disadvantages.
